Are you a carer for a loved one with dementia who has received hospital treatment?
The National Audit on Dementia would like you to complete a short questionnaire so it can assess the quality of care and communication you think they received. The findings will be used to help shape future care.
The questionnaire can be answered by informal carers, family, friends or professionals like key workers.
You can download the questionnaire at here or ask for a hard copy at the reception desk of the ward you are visiting.
