Parkinson’s Disease

Who is the service for?

The service is for individuals who have been diagnosed with Parkinson’s Disease and are registered with a GP located within the London Borough of Brent.

What does the service offer?

  • Nursing assessment at admission and follow up appointments with individualized care plans 
  • Information and education about Parkinson’s Disease and self-management
  • Providing advice and information about medical treatment
  • Referrals or recommendations to other services and agencies that can provide support  

The service is available from 08:30-16:30, Monday to Friday, excluding bank holidays.

Patients will be offered Nurse led clinics appointments and Home visits/ Care home.

 How can someone be referred?

Via email lnwh-tr.referralsparkinsonsdisease through primary and secondary Health Care Professional workers (GP, Consultant, STARRS Team, District Nurses etc.)


Out of hours

If you are unwell and the Parkinson's Nurse Specialist is not available after 16:30  you should contact your GP, NHS 111 (simply dial 111 to speak to an operator who will be able to help you), or the Parkinson's UK helpline on 0808 800 0303.
