About the service

Our specialist health visiting service looks after children who have additional or complex health needs.

The team are part of the Child Development Team (CDT) based at Carmelita House, and work with our community paediatricians.

We offer specialist support to children between the ages of 0 and 19, as well as to their families. This includes:

  • signposting families to other services that might help your child, and make referrals to other services when your child needs them
  • running regular sleep awareness sessions to support parents of children and young people who experience difficulties sleeping
  • seeing your child at an appointment if they have received a diagnosis from the community paediatrician (such as autism or Down Syndrome)
  • working closely with other professionals, such as Early Start Locality teams, Special Educational Needs Assessment Service (SENAS)
  • contributing to Education and Health Care Plans when your child requires this
  • contributubg to multidisciplinary assessments for your child
  • meeting with patients who have concerns relating to their child’s development.
  • advocating for children who have additional needs at meetings and forums (such as the Early Years Placement Panel and Parents of Ealing Self-Help Training Scheme group).

Referrals

We will accept referrals from any professional who is involved with working with a child's family.

We accept referrals for children who:

  • have additional needs
  • have an Ealing GP.

You can refer to our service through:

  • the weekly Child Development Team referral meeting, using an Early Help and Assessment Plan (EHAP)
  • a sleep referral form (for children requiring the sleep group only).
