Here at London North West Healthcare NHS Trust we are committed to ensuring the safety and welfare of all our patients. For people with learning disabilities we are dedicated to making ‘reasonable adjustments’ to ensure that they have a good experience when they visit our hospitals. Our aim is to improve the way hospitals treat people with learning disabilities.
We have a health passport for people with a learning disability for ages 14 to 18 and 18 and over. This should be filled in by a carer or family member, along with the person who has special needs. This will give us information about their care in general and will help when they come in to hospital. It tells our staff how best to communicate with the person based on their needs, helps us to provide specific support while the person is in hospital and lets us know if the carer should be involved in their care.
