Formal complaints

If our PALS team has been unable to resolve your issues or if you feel your concerns are best dealt with as a formal complaint, then you can put them to us in writing. Please give as much information as possible about the incident or episode.

Your letter should be addressed to:

The Chief Executive
London North West Healthcare NHS Trust
Northwick Park Hospital
Watford Road

We will aim to acknowledge your letter within three working days, providing you with a date by which we aim to complete our investigation. If for any reason our investigation takes longer than this, we will write and let you know.

Remember that if you are writing to us on behalf of someone else, please enclose written consent from them stating that they are allowing you to act on their behalf.
