Transfer of community services

On 1 August 2021, all adult and children’s community services currently provided by us will move to two new NHS providers:

  • Central London Community Healthcare NHS Trust will provide all adult and children’s community healthcare services in Brent and all adult community healthcare services in Harrow.
  • Central and North West London NHS Foundation Trust will provide all of Harrow’s children’s services.

Why Brent and Harrow community services are transferring

Our out of hospital (community) services have undergone major changes in recent years. This has led to a reduction in the size and profile of our community services.

Key partners, including the North West London Integrated Care System and Brent and Harrow local authorities, agree that a change in NHS provider will allow community services to concentrate on implementing North West London’s out of hospital services. It will also allow us to focus on our acute and specialist services.

The new community service providers

Central London Community Healthcare NHS Trust (CLCH) is a dedicated community health services provider delivering services across 11 London boroughs and Hertfordshire. They already provide falls prevention, health visiting and school nursing services in Brent and most of adult community healthcare services in Harrow. 

Central and North West London NHS Foundation Trust (CNWL) is also a dedicated community services provider. They provide the 0-19 service (health visiting and school nursing service for 0-19-year-olds) and the Children Looked After service in Harrow as well as children and adolescent mental health services. Both CLCH and CNWL will write to patients with more information after the services transfer over from us.

Will patients see any changes in the care and support they receive?

Our priority is to ensure that everything is in place for a smooth transfer of services on 1 August 2021. This will mean that patients, service users and carers see no difference in the NHS care and support that they receive.

Those of our who are staff affected by the changes will transfer to the new providers on 1 August 2021. This means that after the transfer it is likely that you will continue to see the same member of staff as before.

Patients who receive a prescription for medication from the Child Development Team will continue to do so. However, after 1 August, the prescription will need to be taken to a local community pharmacy (chemist) to be dispensed. Please take the prescription to the community pharmacy at least one week before the medication is due to run out. You must not take your prescription to the Northwick Park Hospital pharmacy as they will no longer be able to supply your medication.

What will happen to patient records?

On 1 August 2021, patient records will be transferred from LNWH to the new NHS care providers. This is standard practice where there is a change of NHS service provider and will enable continuity of your care by the new provider. 

What if I do not want my patient record to transfer?

The Data Protection Act 2018 and General Data Protection Regulation provide patients with specific rights. One is the right to object to an organisation processing personal information. For the transfer of Brent and Harrow community services, you have the right to object to your patient record being transferred to the new provider.

However, it is important to know that, if you tell us not to transfer your patient record, the new NHS provider will not have important historical information relating to your care. This will create gaps in your patient record and may slow the future provision of your NHS care.  Any gaps in your record could also pose a risk to your future care.
