Our district nursing service provides nursing care for housebound patients living in the borough of Ealing. Our service consists of Community Nurses/District Nurses and Advanced Community Practitioners.
How does the service work?
The service is accessed via a Single Point of Access (SPA), contact no. below. The service is available 365 days a year from 8:30am to 5pm and from 4:30pm to 11pm for the Twilight Service. The telephone service is managed locally which means that to contact District Nurses and Advanced Community Practitioners you only need to call one telephone number to reach them.
The service accepts referrals and messages from the public, voluntary sector and health and social care professionals.
Why should I use this service?
This is now the only way to contact District Nurses and Advanced Community Practitioners within Ealing.
What happens when I make a call?
When you call into the service you will be asked a number of questions in order to identify the patient in question and the reason for calling.Your call will then be passed on to the appropriate District Nursing team or Advanced Community Practitioner straight away.
When will I hear from the District Nurse or Advanced Community Practitioner service?
If your call is received before 2pm, you will receive a telephone call by 4:30pm the same day. If your call is received after 2pm, then you will receive a telephone call by 10am the following day. However, if your call is categorised as urgent, you will receive a telephone call and a visit the same day.
