Brent children’s specialist community nursing service

 

The Brent Children’s Specialist Community Nursing Service provides a home nursing service for children from birth up to the age of 18 years (up to 19 years for children with special needs), offering skilled nursing care, professional advice, support and teaching for parents, children and carers to self-manage their conditions. The aim is to enable children with acute and chronic illness or disability to live with their own families, as independently as possible.

The service will also provide the following: 

  • Supporting inclusion by training schools, respite care unit and nursery staff
  • Reducing hospital admissions and re-admission
  • Supporting children and their families in palliative care and end of life
  • Caring for technology dependent children
  • Advocating for children and their families
  • Working in partnership with families.

Referral to the service
The service will take referrals for any child with a specific nursing need that lives in Brent and has a Brent GP. Referrals are accepted from acute paediatric units, tertiary centres, accidents and emergency departments, the acute assessment units, GP’s, schools, other health care professionals and parents.

The Brent Children’s Specialist Community Nursing Service will do home visits, school visits and we also run nurse led clinics. We also train school staff or welfare assistants in the nursing management of children within the school setting.
